We Provide Services For:
Adults above the age of 18 and older adults above the age of 65 who have physical disabilities, conditions or whose behaviour can be challenging, age-related illnesses, life-limiting conditions, long term conditions, complex care needs. The clients we support may require help and support to access the community to enable them to live as independently as they can in their own home. The type of care and support will be tailored to the individual’s needs and their person-centred support plan.
